We utilize electronic appointment confirmation and reminders. Telephone confirmation of appointments is a courtesy we offer for those who choose not to use email. However, we cannot guarantee that we will reach every patient.
Many patients need our services and missed appointments inconvenience everyone. Please mark your appointment on your personal calendar. We request you provide a minimum of 24 business hours notice for all appointment changes. Advance notice of appointment changes ensures we have the opportunity to offer the appointment time to other patients. A charge may be made for failure to keep an appointment when adequate notice is not provided.
As a courtesy to all of our patients, late arrivals may need to be rescheduled. If you arrive late, we reserve the right to shorten or reschedule your appointment.
Check in time is 10 minutes prior to your scheduled appointment time. Upon arrival, please be prepared to present your insurance card, if applicable.
Cancellation, Late, and Missed Appointments
Appointment times have been arranged spcifically for you. Out of respect and consideration to our staff and other patients, please plan accordingly and arrive on time.
*Note: All cancellations with less than 24 hours notice will incur a $65 fee per hour reserved for the missed appointment. If you arrive more than 15 minutes late for your appointment, it will likely be rescheduled and will incur a $65 fee. If time permits we may offer you a shortened appointment.
Patients are seen on an appointment basis only. Please call if you are having a dental problem so that an appropriate appointment time can be arranged. Patients of record in need of emergency dental care during non-office hours should call our office to obtain the emergency contact phone number for the dentist on call.
Be sure to bring your dental benefit card with you to your appointment. Please note that your dental plan may be with a different carrier than your medical plan. If you were not issued a card, please be prepared with the following information.
- Insurance carrier name and phone number
- Claims mailing address
- Group/Plan number
- Subscriber ID number
- Subscriber birth date
As a courtesy, we are happy to submit a claim for services to your insurance plan. If insurance benefits are delayed or your carrier fails to pay for services within 30 days, your personal payment is expected. All charges are your responsibility from the date the services are provided. We will gladly provide you one additional copy of the claim form to resubmit for your personal reimbursement if needed.
Forms of payment accepted are personal checks, cash, VISA, and MasterCard.

Any portion of an account balance beyond 60 days incurs a service charge of 1.5% per month. Returned checks incur a handling fee of $25.
We encourage anyone having temporary financial problems to contact us immediately so we can assist you in the management of your account.

While waiting in our reception area, WI-FI is available. Our receptionist can provide you the access code.
